SIGN-UP FOR U.S. AGENT PROGRAM - FOREIGN FOOD ESTABLISHMENTS
In order to sign up for mdi’s U.S. Agent Program for Foreign Food
Establishments you need to fill out and send the following forms to us:
1. Retainer Agreement
2. U.S. Agent Information Form
However prior to filling out these forms you will need to contact us,
so that we can customize the forms to your requirements. You can contact

Schwartz) to discuss your requirements.
Below we have provided you examples of our Retainer Agreement and
U.S. Agent Information Form to give you a sense for the type of
information we will need from you.
