SIGN-UP FOR U.S. AGENT PROGRAM – PHARMACEUTICAL AND MEDICAL DEVICE
MANUFACTURERS
In order to sign up for mdi’s U.S. Agent Program for Pharmaceutical
or Medical Device Manufacturers you need to fill out and send the
following forms to us:
1. Retainer Agreement
2. U.S. Agent Notification Letter
However prior to filling out these forms you will need to contact us,
so that we can customize the forms to your requirements. You can contact

Schwartz) to discuss your requirements.
